When a child is born in Ontario their birth must be registered with the Government of Ontario through the Office of the Registrar General, within 30 days. A birth certificate is a "foundation identity document" that can be used as proof of identity and is required to apply for other forms of identification, such as Social Insurance Numbers or health cards.
Birth registration in Ontario is a two-step process:
- Statement of Live Birth Form, is completed by parents of newborns and submitted to Service Ontario
- Notice of Live Birth Form, is completed by qualified medical staff and submitted to Service Ontario
Service Ontario in co-operation with Service Canada offer parents a fast and easy approach to register your child's birth online and apply for a birth certificate and Social Insurance Number at the same time. Registering using a paper process is also an option. The signature(s) of each parent named on the statement must be on the form.
Where parents opt to register a child using the paper process, the statement must be sent directly to the Office of the Registrar General at 189 Red River Road, PO Box 4600, Thunder Bay, Ontario P7B 6L8. To complete the paper format, a blank Statement of Live Birth may be accessed online at Service Ontario.
